• What is the name of the activity or the service you would like to request? Please include here all the service's options you like to request.
• How many days do you have available to enjoy the experiences?
• What exact dates does your group have to do the experiences you are about to request? If you can include the hours you want to start your experiences by day and how much time you have available each day will be appreciated.
• How big is your group? Number of people.
• Please provide enough information of your group, answering the following questions:
• How many of them are between the ages (16 years old or more), (6 to 15 years old), (0 to 5 years old)?
• Health Conditions and Allergies we should be aware of?
• Does anyone have any type of disability (functional diversity)?
• Do you need any reasonable accommodation? Explain.
• Will you be accompanied by a person who will assist you in sign language interpretation?
• Please include the information here of the person who will assist you with sign language. Include the age and conditions of the person.
• Does your group need transportation? Can you tell us where your pick up and drop off location is? Also, include every detail of your group that we need to consider. Example: Car Seat
• Which language do you want during the service? English or Spanish?
